Please be sure all members in your organization are advised. I have received a few inquiries regarding this matter that requested the following background/information: 1. The initiative to reinstate the NASA "meatball" vs. the "worm" began 7 years ago. (A memorandum to Officials-in-Charge and Center Directors mandating the changeover was issued.) The changeover included revisions to NASA Forms, NASA Headquarters Forms, letterhead, envelopes, etc. 2. NASA Form 422 Signature/Incoming Tab was revised initially in May 96 and again in Nov 96 (both revisions were made to reflect the "meatball") 3. NASA Form 423 Concurrence/Background Tab was revised in November 96 (revision was made to reflect the "meatball") 4. Technically, pursuant to 14 CFR, 1221.1 the former (worm) tabs are obsolete. 5. The tabs (reflecting the "meatball") are available and can be ordered. Fill out NASA Form 4 and send it to GSFC Stock, Code 239.9. Be sure to reference both the NF and Stock Nos. as listed below.
